자가면역 췌장염 (autoimmune pancreatitis)

정의: 자가면역에 의해 유발되는 만성 췌장염의 한 형태로 특징적인 조직병리 (LPSP) 및 타 장기침범을 동반하는 질환

임상양상 및 특징

• 증상: 황달, 체중감소, new onset DM

• 혈중 IgG4 수치 증가 (IgG4 related disease의 한 일환으로 보기도 함)

• 담관, 침샘, 후복막, 임파선, 갑상선, 신장, 폐, 간 등의 섬유화 염증 동반

• 스테로이드 치료에 반응이 좋음

조직학적 특성

• Lymphoplasmacytic sclerosing pancreatitis (LPSP): periductal lymphoplasmacytic infiltrate, storiform fibrosis, obliterative phlebitis

  

영상 소견

• CT: diffuse/focal enlargement of pancreas, inflammatory rim (capsule sign)

• PET-CT: diffuse uptake

• ERCP/MRCP: bile duct strictures (PSC와 유사한 형태를 보일 수 있음)

진단 기준: HISORT criteria (histology, imaging, serology, other organ involvement, response to glucocorticoid therapy)

치료: glucocorticoid
