염증성 근육병증

• 종류: 다발근육염(PM), 피부근육염(DM), 봉입체근육염(IBM) 등

  
  

• 임상양상

  • 근위부>원위부, 대칭성/진행성 근력저하

    • “몸일으키기, 계단오르기, 머리빗기 등”

  • 피부근육염: 특징적 피부소견 

    • Heliotrope rash (eyelid erythematous discoloration + periorbital edema)

    • Gottron’s sign (erythematous rash over extensor suface of joints)

    • V sign (rash on sun-exposed ant. neck and chest)

    

  • Mechanic’s hand: 손바닥과 손가락 측면의 과각화와 갈라짐

    

  • 암과의 관련성이 있기에 underlying malignancy 감별 필요

  • ILD 발생 위험 증가 -> 가장 흔한 합병증, mortality 높음

  • 심장 침범: 비교적 드묾, myositis, 전도장애, 심부전

    
    

• 진단

  • 전형적 피부근육염은 임상증상 + 검사실 소견만으로 진단 가능

    • 검사실 소견

      • 자가항체: Anti-Jo-1 Ab (PM > DM), Anti-SRP Ab (PM), Anti-Mi-2 Ab (DM)

      • CK상승 (근육 파괴의 증거)

      • EMG: low ampilitude, polyphasic, abnormally early recruitment 등

  • 그 이외에는 감별을 위해 근육생검이 필요

    
    

• 치료

  • 스테로이드 (반응 없으면 면역억제제 추가)
